The American Association of
Insurance Services (AAIS) has created two declaration form samples for use with
either the CU 0001 or the CU 0002. These are just two of many possible formats,
as each insurance company typically develops its own declarations. They include
all essential information to identify the coverages offered by the policy.

The CU 0051 Declaration and CU
0052 Declaration are quite similar, differing only in the Limits of Insurance
and the Schedule of Underlying Insurance sections.

NOTE: When coverage extends to multiple
named insureds, they are typically listed or named on a separate endorsement.
Ensuring the mailing address is accurate and complete is essential so the first
named insured receives all mailings promptly.
NOTE: Other includes corporations.
A brief description of the named
insured's primary business is entered in this area.
This includes the effective and
expiration dates, as well as the start and end time. The sample policies
indicate a time of either 12:01 am Standard Time or 12:00 noon Standard Time.
NOTE: Coverage typically starts at
12:01 a.m. Eastern Standard Time at the mailing address location. It’s
very important to review the specific policy declarations page for the exact
time.
NOTE: The retroactive date applies only
to claims-made coverage. The insured often prefers it to be the earliest date
coverage was in place on a claims-made basis. However, insurance companies
generally prefer later retroactive dates so they can limit their liability
exposure.
This section states that this
declaration supersedes all previous declarations. It explains that coverage
applies only to losses, accidents, offenses, or incidents occurring during the
policy period. It also clarifies that each one-year term ending on the
anniversary date is considered a separate policy period for coverage written on
a continuous basis.

Self-insured Retention is in a
separate section on the CU 0052 Declarations. It functions much
like a deductible. It is the dollar amount the named insured pays before this
insurance kicks in. However, this limit only applies to losses covered by the
umbrella but not covered by the underlying insurance.

the signature of the authorized representative who countersigns the policy and

officers can also be placed in the space provided below this section, on the
first page of the coverage form, or elsewhere, at the insurance company's
discretion.
